The application period for the February 2027 NPLEX administration will be open October 1-30, 2026.

Name Change Form

It is your responsibility to ensure that NABNE has current information regarding your legal name. If the first and last names on your acceptable identification document do not exactly match the first and last names you have provided in your NPLEX application or on this form, you may not be admitted to take the examination.

If you have had a legal name change since you last applied to take an NPLEX Examination, you must notify NABNE by using the Name Change form. When you complete this form, you will be required to scan and upload a copy of:

  • The documentation (e.g., marriage license, name change order, etc.) that verifies your official name change.
  • An acceptable identification document issued under your new name that contains your photo and signature (e.g., unexpired passport, driver’s license, state identification card, or military identification card )

Both the first and last names on both items must exactly match the first and last names you provide on this form and your NPLEX application.
